As of March 2016, the San Antonio Metropolitan Health District revised its restaurant scoring system. It is now based on a 100-point demerit system and features a number score at the top and bottom of the inspection report.

A placard featuring the number score and a letter grade is also available upon request from the food establishment. Inspection reports must be posted in public view. Posting the placard with a letter grade such as 95/A, for example, is solely at the restaurant's discretion.

The scoring is now as follows:
90-100 = A
80-89 = B
79 or lower = C
